    Established in 1948, The Mission of ASCEND Mental Wellness is to improve the quality of life for those affected by mental illness and to promote the awareness and importance of mental well-being in the community.

    ASCEND is currently looking to fill the position of Supportive Housing Assistant Coordinator at our Housing First location. This individual will assist the Program Coordinator in overseeing the day to day operations of a 18 unit facility of permanent, supportive apartments as well as transitional/emergency apartments for individuals who are homeless or unstably housed.

    Individuals must be well motivated and committed to making a difference in people's lives. Good listening, writing and verbal skills required. The ideal candidate will be comfortable working independently.

    Responsibilities include, but aren't limited to, the following:

    • Reconcile petty cash and program expenses weekly.
    • Responsible for completing Support Plans and Billable notes in a timely manner
    • Assist with all billing matters
    • Assist with developing monthly menus
    • Assist with developing staff schedules and ensuring appropriate staff coverage
    • Assist with facilitating program orders, including but not limited to, program supplies
    • Assist in setting up new EHR for all individuals referred to programming.
    • Assist in internal audits.
    • 1:1 supervision with Residential Specialists
    • Audit files as necessary
    • Maintaining individual’s electronic charts, including but not limited to, scanning and uploading needed documents.
    • Ensuring that staff complete their job duties and following up with staff as necessary.
    • Work with Coordinator team to ensure environmental repairs are completed in a timely manner.
    • Assure communications of resident hospitalizations to appropriate parties.
    • Carry a caseload as needed to assist the program, and complete the documentation needed.
    • Provide additional services for the program as needed
    • Oversee tracking of Support Plans and Billable notes
    • Assist with onboarding and training of new staff
    • Assist in the coordination of house/community meetings.
    • Assist in program certification/re-certification processes as required by CARES, HMIS, HUD, OMH, OTDA, & NYSSHP
    • Coordinate and assist in Rep-Payee services for tenants
    • Arrive on time for all scheduled shifts
    • Maintain a safe environment for all clients and staff.
    • Maintain effective communication with other staff members and supervisors.
    • Preserve human dignity and confidentiality of all clients and act in the best interest of those served through the Agency’s programs and services.
    • Always assure quality of work.
    • Adhere to all laws and regulations applicable to the Agency’s business.
    • Adhere to the Agency’s Corporate Compliance Program and established policies and procedures.
    • Always positively represent the Agency within the community.
    • Have adequate computer skills, or more refined for specific job titles.
    • Actively participate in scheduled team/staff meetings as scheduled by supervisor or as indicated by the program’s services.
    • Actively participate in Agency sponsored staff development and in-service educational programs as deemed necessary by supervisor, including mandatory training assigned per job title.
    • Should be compassionate and demonstrate a commitment to helping people with mental illness and/or co-occurring disorders

    Qualifications/Requirements:

    • High School Diploma, or equivalent, with at least five years’ experience working in the human services field. An Associates degree in the human services field may be substituted for two years’ experience.
    • Valid NYS driver’s license acceptable to Agency insurance standards.
    • Must have the ability to work independently, have good problem-solving skills and can work as part of a multi-disciplinary team.

    Job Type: Full-time

    Pay: $42,000.00 - $52,000.00 per year
