Rehabilitation Counselor/Direct Support Professional/Job Coach/Placement Manager
Job Description
PREFERRED QUALIFICATIONS
B.S. or B.A. degree required, and two years of experience working in the direct care of individuals in a mental health program or in a vocational program, which serves people with developmental disabilities and/or mental health disorders.
Reports to Assistant Executive Director, who reports to the Executive Director
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
PRE-VOCATIONAL TRAINING
- Learn the tasks and procedures of each pre-vocational training unit.
- Develop a thorough knowledge and understanding of member goals to apply in unit.
- Develop positive and trusting relationships with members.
- Learn and practice clubhouse model principles.
- Develop training techniques and strategies for members.
- Role model professional work habits within units.
JOB DEVELOPMENT/PLACEMENT MANAGEMENT
- Encourage members to explore work opportunities.
- Assist members with the completion of employment applications.
- Assist members with interviews and securing a job.
- Assist members through the hiring process.
- Provide on-the-job training for individuals entering the workforce.
- Provide travel training to assist member in getting to and from work.
- Develop relationships with management, supervisors and co-workers.
- Assess vocational abilities and need for further training.
- Provide follow-along services to individuals at job sites.
- Effectively and professionally, deal with employer needs and concerns.
DOCUMENTATION OF SERVICES
- Develop appropriate goals for primary caseload.
- Accurately report progress on goals.
- Responsible for timely submission of service plan reviews.
- Responsible for the maintenance of all required chart work for primary caseload.
- Accurately report observations regarding members in staff meeting.
- Documentation of services as required by OMH, OMRDD and VESID.
OTHER FUNCTIONS
- Encourages members’ self-advocacy and the development of natural and community supports.
- Models positive relationships with staff, members and the larger community.
- Presents a professional demeanor at work and in the community.
- Recognizes members’ strengths and helps members work from and build upon their strengths.
- Ensures confidentially of all client records and information.
- Complies with all reporting requirements of OMH, OPWDD, ACCES-VR and MEDICAID.
- Represents the agency in such a way that fosters an understanding of the goals of the Clubhouse Model of Rehabilitation.
- Carry out other duties as assigned by Executive Director and/or Assistant Executive Director.
